Coordinator Please note that it is your responsibility to obtain all necessary authorizations AND referrals if your insurance carrier requires them (many require both a referral AND authorization) as well as to follow up on claims that are greater than two months old. All copies are due at the time of your visit. Insurance coverage for infertility treatment is very inconsistent, so please feel free to speak to the Referral Coordinator or Business Manager if any questions arise regarding insurance reimbursement issues. We accept a variety of commercial, PPO and HMO insurances. To find out if yours is included, call the office at (804) 285-9700 and speak to our Referral Coordinator. |
